Systems that make stuff up

We trusted IT systems because they didn’t make stuff up. This assumption just broke.

A database returns what you stored.
A compiler turns your code into exactly what you wrote.
Encryption gives back the original or fails completely.

These systems break loudly. They throw errors. Something turns red. You find out.

LLMs, agents, RAG pipelines, AI transcription, they don’t retrieve, they generate. And generation means they can hand you a confident, fluent, perfectly-formatted answer that is simply wrong.

No error. No red light. The API returns 200 OK.

The dangerous part isn’t that AI fails. It’s that it fails silently, while everything you built to catch failure was designed for the old kind.

The loud failures become incidents. The silent ones become your new normal.

Which of these failures would ever show up on your dashboards?
